Chronic Care Management

• At least 2 chronic disease states
• Chronic: greater than 3 months of life and ends typically at death
• Time spent on the patients monthly collectively adds up
• Time from pharmacy, medical staff and providers
• Great service for our patients that do not come in every month
  

Diabetes Self-Management Class

• Occurs once monthly at our Monroe office
• Patients with uncontrolled diabetes
• Typically A1c>8% but can be lower
• New and “seasoned” diabetic patients
• Pharmacist driven - Dr Wynn present for classes
• Breakfast provided that is diabetes friendly
• 1-2 hour discussion in a group setting and individual time with the provider
